Baby Plate Orders

How to Order

Each of our baby plates is hand-painted and lettered. Please use care with spelling and numbers. I cannot change them later (they’ll be set in stone, as it were).

Enter only the details you want on the plate. If you just want the baby’s name, animal and birthdate, just leave the other lines blank.

You may either use the form below or call (707-942-0216) with the following details (that you want on the plate):

  • Child’s Name
  • Length
  • Weight
  • Time of Birth
  • Date of Birth
  • Animal Decoration
  • Band Color (Choices are GREEN, BLUE, RED, or NONE)

Payment

If you are paying by credit card, please call with your information.

  • Plate shipped UPS, continental US $80.00
  • Plate picked up at the pottery $60.00 plus sales tax.
  • Please allow 8 weeks, longer if we have closed for any reason.
