To order, ﬁll out the form below and press the submit button. This form does not constitute an order. It is merely a tool to produce a legible order that will be commisioned on your behalf once payment is received. You will have a chance to correct any mistakes.

Animal decoration
Color of band	green blue rust
Leave a blank in any space where you don't want extra information (such as "length") to appear on a finished piece. If you don't want a color band around the edge, then leave the "Color of band" field blank.

After pressing Submit, you'll be redirected to a filled out form which you can print. READ IT OVER CAREFULLY TO MAKE SURE EVERYTHING IS CORRECT.
